    1. A Completed Visa Application Form

    2. A Passport Copy

    护照的有效使用期至少为六个月。
    Its period of validity should be available for six months at least.
   
   
   
   
    3. A Notarized Non-criminal Certificate
    经公证的无犯罪记录证明（须申请人在其本国外交部认证和玻利维亚驻华使馆认证）
   
   
    近期在其本国无犯罪记录证明，该证明须公证及双认证。
    A current non-criminal certificate issued by Police Office of one’s native country should be notarized. The notarized certificate should be also authenticated by the Ministry of Foreign Affairs of one’s native country and the Embassy of the Republic of Bolivia.
   
   
   
   
    4. A Physical Examination Certificate
    经公证的体检证明（须申请人其本国外交部认证和玻利维亚驻华使馆认证）
   
   
    近期在其本国体检证明，该证明须公证及双认证。
    A physical certificate issued by hospital of one’s native country should be notarized. The notarized certificate should be also authenticated by the Ministry of Foreign Affairs of one’s native country and the Embassy of the Republic of Bolivia.
   
   
   
   
    5. Diploma and school report card Certificates
    经公证的毕业证书和成绩单（须申请人其本国外交部认证和玻利维亚驻华使馆认证）
   
   
    该证明须公证及双认证。
    The diploma and school report card should be also authenticated by the Ministry of Foreign Affairs of one’s native country and the Embassy of the Republic of Bolivia.
   
   
   
   
    6. A Authorization Letter from parents
    经公证的父母授权书（须申请人其本国外交部认证和玻利维亚驻华使馆认证）
   
   
    申请人父母开具的同意其到玻利维亚留学的正式公函，该授权书须公证及双认证。
    The official letter from applicant’s parents approval their children to study abord to Bolivia, it should be notarized. The authorization letter should be also authenticated by the Ministry of Foreign Affairs of one’s native country and the Embassy of the Republic of Bolivia.
   
   
   
   
   
   
   
   
    7. A Admission Letter from University of Bolivia
    经公证的大学录取通知书（须申请人其本国外交部认证和玻利维亚驻华使馆认证）
    该证书须公证及双认证。
    The Form Admission Notice should be also authenticated by the Ministry of Foreign Affairs of one’s native country and the Embassy of the Republic of Bolivia.
   
   
    注：申请人申请的玻利维亚学校必须是合法的！
    NOTE: The Bolivia school that the applicant applied must be legitimate!
   
   
   
   
    8. A Certificate of Assets（Certificate issued of bank is preferred）
    在其本国的资产证明（银行证明最佳）
   
    通过相关资料证明在其本国的资产，须对资产证明进行公证及双认证。
    Relevant files issued by one’s native country to certify that the applicant has adequate assets, it should be notarized and authenticated.
   
   
   
   
    9. A Certificate of Vaccination against Yellow Fever
    黄热病接种证明
   
   
    有效期内的黄热病接种证明
